Young kids could be irritable, often febrile, and incessantly pull at their ears, but they might even be utterly asymptomatic. Older kids and adults note ear pain, impaired listening to, and occasionally otorrhea. A mildly erythematous tympanic membrane is seen with a small purulent effusion within the middle ear. The center ear is crammed with purulent materials behind an erythematous, bulging tympanic membrane. Most children over 2 years qualify for observation for forty eight hours prior to initiation of antibiotics. A clear, amber-colored effusion with a single air-fluid level is seen in the middle ear behind a traditional tympanic membrane.
